The client is a public community hospital with the primary goal of delivering healthcare to safety-net communities. Opening in 2015, it has grown beyond its original hospital footprint, adding emergency rooms to include ambulatory clinics and community outreach, all of which are focused on providing quality healthcare to both underinsured and uninsured patients.
The hospital is committed to the health of its community, and part of that population is the homeless. With a goal to provide equal care to that found in affluent areas, it has made Population Health a priority by building evidence-based care models, disease-specific programs, care coordination, and the identification of clinical improvement opportunities across the continuum of care.
To support this goal, the hospital collaborates with the California Department of Health Care Access and Information (HCAI) to collect, manage, analyze, and report information about California’s healthcare infrastructure and patient outcomes. In the process, the client adheres to the mandate to deliver homelessness as a Population Health data set in the form of a flat file, as specified in the California Code of Regulations for HCAI.
